How to Get the full suit of Juggernaut armor in Dragon Age: Origins

The Juggernaut armor is the best armor in the entire Dragon Age: Origins game, especially when you have the full suit! Three pieces are easy to find - disturb the Ancient Tombstones in the Brecilian Forest, and then defeat the monsters that spawn.
